This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who we are

This notice applies to McAlpine Chiropractic Group, P.C. (doing business as McAlpine Chiropractic), its chiropractors, massage therapists, and staff. We are required by law to keep your health information private, to give you this notice of our legal duties and privacy practices, and to follow the notice currently in effect.

How we may use and disclose your health information

Treatment. We use your health information to examine and treat you, and we may share it with other health-care providers involved in your care — for example, your primary care doctor, an imaging center, a physical therapist, or a specialist we refer you to.

Payment. We use and share your information to bill and collect payment for your care — with your health insurer, an auto no-fault insurer, a workers’ compensation carrier, or a collection agency if an account is overdue. Michigan law requires us to give an auto no-fault insurer a written report of your history, condition, treatment, and charges when it asks.

Health-care operations. We use your information to run the practice — quality review, training, audits, business planning, and the systems we use to keep records, schedule, and bill, including our electronic health record and the service providers that support it. Service providers who handle your information sign agreements requiring them to protect it.

Appointment reminders, treatment options, and health information. We may contact you by mail, phone, voicemail, email, or — only if you have separately opted in — text message, to remind you of appointments, follow up on your care, or tell you about treatment options and health services we offer. You may ask us to use a different method or stop.

Family and others you involve. We share information only with a family member, friend, or caregiver you have named on your intake paperwork or whom you bring with you and allow to be present, and only what is relevant to their involvement in your care or payment. You can change who that is at any time. If you are unable to tell us, for example in an emergency, we use professional judgment to share only what is needed.

As required or permitted by law. We may share your information without your authorization when the law requires or permits it, including: public-health reporting; reporting abuse, neglect, or domestic violence; health oversight agencies such as licensing boards; court orders, subpoenas, and lawful discovery requests; law-enforcement purposes as permitted by law; coroners and medical examiners; workers’ compensation programs; to prevent a serious threat to health or safety; specialized government functions such as military or national security; and to the Secretary of Health and Human Services to investigate our compliance.

Uses that require your written authorization

We will not use or share your information for marketing, sell your information, or use your photo, name, or testimonial in advertising without your separate written authorization. Any use or disclosure not described in this notice requires your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Your rights

  • See and get a copy of your records. Ask in writing; we will provide a copy, usually within 30 days, in the form you request if we can readily produce it (paper or electronic). We may charge a reasonable cost-based fee allowed by Michigan law.
  • Ask us to correct your records. Ask in writing and tell us why. We may deny the request in certain cases; if we do, we will explain in writing and you may file a statement of disagreement.
  • Get a list of whom we have shared your information with (an accounting of disclosures) for the six years before your request, other than disclosures for treatment, payment, operations, or ones you authorized.
  • Ask us to limit what we use or share. We are not required to agree, except: if you pay for a service in full out of pocket and ask us not to tell your health insurer about it, we must agree unless the law requires the disclosure.
  • Ask for confidential communications — for example, contacting you only at a certain phone number or address. We will accommodate reasonable requests.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Be notified of a breach of your unsecured health information.
  • Choose someone to act for you. A legal guardian or someone with medical power of attorney may exercise your rights on your behalf.

Our responsibilities

  • We must keep your health information private and secure and follow the duties and practices described in this notice.
  • We will tell you promptly if a breach may have compromised your information.
  • We will not use or share your information other than as described here unless you tell us we may in writing; you may change your mind at any time by telling us in writing.

Changes to this notice

We may change this notice and the changes will apply to all information we hold. The current notice is posted in our office and at mcalpinegroupllc.com/notice-of-privacy-practices/, and a copy is available at the front desk.
