What is McAlpine’s auto-injury treatment program?
Our auto-injury program is a structured, combined-modality treatment plan for patients recovering from motor vehicle accidents. It covers initial evaluation against established clinical red-flag guidelines, a phased treatment protocol (chiropractic adjustments, non-surgical spinal decompression, Class IV laser therapy, soft-tissue work, and rehabilitation), insurance verification under Michigan no-fault, and coordination with your family physician, attorney, imaging center, or specialist when a case calls for it. Most patients are seen within the same week of contact, including post-crash referrals.

The first 72 hours after a crash — what to do
The first three days after a motor vehicle accident set the trajectory of your recovery. Adrenaline and inflammatory cascades suppress pain immediately after impact, then ramp up over the following 24 to 72 hours. Patients who delay evaluation often present a week later with significantly worse symptoms.
- Rule out emergency injuries first. If you have severe headache, neurologic symptoms (numbness, weakness, vision changes), confusion, severe chest or abdominal pain, or loss of consciousness during the crash — go to the ER first. We coordinate with hospital teams for post-discharge follow-up.
- Document the accident. Photos of vehicle damage, the police report, and contact details of all parties and witnesses. Your auto insurer will need these for no-fault claim processing.
- Call us within 48 to 72 hours. Even if you “feel fine” — call 616-392-7031. We will get you scheduled for evaluation, often the same week. Early baseline documentation matters clinically and for insurance.
- Do not start aggressive activity. Movement is good. Returning to the gym, heavy lifting, or full work duty before evaluation is not. Light walking and gentle range of motion are fine.
- Do not sign settlement paperwork yet. Insurance adjusters sometimes contact patients early offering quick settlements. Your initial evaluation — what to expect
The first visit takes about 60 to 75 minutes. We do not start treatment until we have completed the evaluation and discussed the plan with you.
- Clinical history — mechanism of injury (rear-end, side-impact, T-bone, single-vehicle), symptoms, when they started, what makes them better or worse, prior injuries, current medications, medical history.
- Red-flag screening — we screen against the ACP 2017, NICE NG59, IFOMPT 2020, and Canadian C-Spine Rule guidelines. If any red flag presents, we coordinate with your family physician or refer for imaging before treating.
- Physical examination — range of motion, neurological screen (reflexes, dermatomes, myotomes), orthopedic testing of cervical, thoracic, lumbar, and any other involved region.
- Imaging decisions — most uncomplicated cases do not need imaging at first visit, per ACR Appropriateness Criteria. If imaging is indicated, we order or refer for it.
- Treatment plan discussion — we explain what we found, what the projected recovery looks like, and what your week-by-week visit schedule will be.
- Insurance verification — we verify your no-fault benefits while you are at the visit so you know what is covered before treatment starts.
The combined-modality treatment program
Auto-injury patients almost always recover faster on a combined approach than on any single modality. Our program sequences five tools in the right order, adjusted to the acuity and presentation of each case.
1. Chiropractic adjustments
Restore joint motion in the cervical, thoracic, lumbar, or sacroiliac regions affected by the accident. We use both manual and instrument-assisted (Activator, ArthroStim) techniques depending on acuity and patient tolerance. 2. Non-surgical spinal decompression
For auto-injury cases with disc involvement — herniation, bulge, annular tear, or radicular symptoms (radiating arm or leg pain). 3. Class IV laser therapy
Reduces inflammation at the cellular level in injured tissue. Particularly effective for the facet joint and ligamentous injury patterns common after whiplash and seatbelt-related shoulder and rib trauma. 4. Soft-tissue work
Graston, Active Release Technique (ART), and targeted manual release for the muscle guarding that develops after impact injury. Muscle guarding that is not addressed becomes the chronic stiffness patients report years after a crash. Massage therapy is also available on-site for complementary work.
5. Corrective exercise and rehabilitation
Targeted home exercises progressed over the treatment plan — deep cervical flexor activation, scapular stabilization, lumbar stabilization, and graded return-to-activity. Most patients receive a written plan with photos or video links so they can follow along between visits.
Michigan no-fault auto insurance — what’s covered
Michigan operates under a no-fault auto insurance system. Under no-fault, your own auto insurance policy pays for reasonable and necessary medical treatment after a motor vehicle accident, regardless of who caused the crash.
- Chiropractic care is covered. Michigan no-fault explicitly covers chiropractic treatment when prescribed as reasonable and necessary for accident-related injuries.
- Coverage details vary by policy — specifically whether you carry coordinated or uncoordinated medical coverage, your deductible, and the medical benefit cap you selected (Michigan’s 2019 PIP reform created tiered options ranging from $50,000 to unlimited).
- We bill your auto insurer directly. You do not pay upfront for accident-related treatment under most no-fault policies. We handle the documentation and submission.
- Treatment must be related to the accident. Pre-existing conditions are evaluated separately and may fall under your regular health insurance.
- Time limits apply. Michigan no-fault typically requires application for benefits within one year of the accident. Sooner is always better.

- Family physician — for short-course anti-inflammatories, muscle relaxers, or steroid tapers when acute pain warrants pharmacology alongside structural care.
- Personal-injury attorney — we work with attorneys experienced in Michigan no-fault when third-party recovery or settlement representation is needed. We can provide referrals.
- Advanced imaging centers — MRI or CT when conservative care plateaus or red flags require structural workup.
- Physical therapy — for complex rehabilitation, vestibular involvement, or post-surgical recovery.
- Pain management specialist — for severe acute pain that benefits from injection-based intervention while structural care continues.
- Neurology or concussion specialist — for cases with cognitive symptoms, persistent vestibular involvement, or suspected post-concussive features.

Frequently asked questions
Do I need a referral from my family doctor to start?
No. Michigan no-fault does not require physician referral for chiropractic care. You can call us directly. We coordinate with your family physician throughout the case.
How soon after the accident should I be seen?
Within the first week, ideally within 48 to 72 hours of the accident. Earlier evaluation gives a clinical baseline before inflammatory and pain cascades peak, and supports your insurance claim with timely documentation.
What if I already started care elsewhere?
We can take over or work alongside another provider. Bring your records — we will review, identify gaps, and either continue what is working or add the combined-modality elements that may be missing.
What if my symptoms started months ago and I never got treated?
Delayed-presentation auto-injury cases are treatable. The tissue has remodeled and there is often layered neural sensitization and movement compensation, but the protocol simply runs longer — typically 8 to 12 weeks of combined care. Michigan no-fault may still cover the case if you are within one year of the accident; we will help you verify.
How long does the typical treatment plan run?
Uncomplicated cases often resolve in 4 to 8 weeks with 2 to 3 visits per week tapering down. Cases with disc involvement, persistent radicular symptoms, or pre-existing factors typically run 8 to 16 weeks. We re-evaluate every two to three weeks and adjust based on progress.
